Insulinoma: anesthetic implications.
نویسنده
چکیده
Insulinoma is a rare functioning endocrine tumor of the beta cells of the pancreatic islet of Langerhans. The clinical picture presented is due to episodes of hypoglycemia induced by increased secretion of insulin by the tumor. Harris in 1924 was the first to suggest the possibility of spontaneous hypoglycemia, and in 1927 Wilder made the first reported preoperative diagnosis of an insulin-secreting tumor.1 W. J. Mayo operated on Wilder's patient and found a pancreatic tumor with metastasis to the liver. The biopsy showed an islet cell tumor which was confirmed several weeks later by autopsy, and an assay of the liver showed large amounts of insulin in the tumor tissue. In 1929, Graham first successfully removed a benign insulinoma and the patient experienced full recovery. 2 It is interesting to find that there are few references in the literature to the anesthetic management of patients undergoing operative removal of these tumors. The purpose of this paper, therefore, is to review the literature and acquaint the nurse anestlhetist with the preoperative and intraoperative management of this rare disorder.

متن کاملClinical Implications of Various Criteria for the Biochemical Diagnosis of Insulinoma
BACKGROUND Among the various diagnostic criteria for insulinoma, the ratio criteria have been controversial. However, the amended insulin-glucose ratio exhibited excellent diagnostic performance in a recent retrospective cohort study, although it has not yet been validated in other patient cohorts. We examined the diagnostic performance of the current criteria of the Endocrine Society, insulin-...
متن کاملIdentification of a calcium-regulated insulinoma cell phosphoprotein as an islet cell keratin
This report describes the cytoskeleton nature of a 60,000-mol-wt protein, P60, previously shown to undergo Ca2+ influx-induced phosphorylation concomitant with insulin release in hamster insulinoma cells. Four lines of evidence suggest that P60 is an intermediate filament protein of the keratin class. (a) As previously described (Schubart, U.K., 1982, J. Biol. Chem. 257:12231-12238), Triton X-1...
